The Registered Nurse performs, organizes, and directs Home Health Care Services with an emphasis on community health education. They build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes.

Responsibilities
Patient Care

  • Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illnesses.
  • Regularly re-evaluates patient nursing needs.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventative, and rehabilitative nursing actions. Administers medications and treatments as prescribed by the physician.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan of care.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
  • Acts as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility to coordinate patient care for assigned caseload.


Communication

  • Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
  • Communicates with community health related persons to coordinate the care plan.


Additional Duties

  • Participates in on-call duties as defined by the on-call policy.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Instructs, supervises, and evaluates home health aide and LVN per agency policy.


Qualifications

  • Must be a Registered Professional Nurse (California) with current licensure
  • Graduate from an approved school of professional nursing (AA, Diploma or BSN)
  • CPR, ACLS, and BCLS Certification
  • Knowledge of OSHA, FDA, and HIPAA compliance
  • Minimum of 1 year experience as a Registered Nurse.
  • Must be a licensed driver with an automobile that is insured in accordance with State/or organization requirements and is in good working order.
  • Self directed and organized, able to work with minimal supervision.
  • Demonstrates excellent observation, problem solving, verbal and written communication skills in English; nursing skills competency checklist.
  • Computer literate preferred.
  • Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.

Physical Therapist (Rio Vista)

Responsibilities

1. Provides physical therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to:

  • Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluate home environment.
  • Assesses muscle strength, mobility, gait, ROM—potential for rehab.
  • Directing physical therapy treatment.
  • Instructing patients and families/caregivers in the use and care of therapeutic appliances.
  • Determining priority needs for physical therapy.
  • Reporting to the physician, the patient’s reaction to treatment or changes in condition.


2. Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.

3. May train patients in the use of prosthetic devices.

4. Identifies patient and family/caregiver needs for other home health services and refers

as necessary.

5. Prepares and submits clinical and progress summaries based on the attainment of goals.

6. Participate in discharge planning for patients.

7. Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.

8. Provide physical therapy consultation to home families/caregivers when indicated.

9. Provides in-service education programs for nursing organization personnel as needed.

10. Participates in peer consultation process.

11. Supervises Physical Therapy Assistants according to organization policy and state regulations.

12. When therapy is the only skilled service, instructs, supervises, and evaluates home health aide care every two (2) weeks.

Qualifications
1. Possesses a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization.
2. CA physical therapy license
3. Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
4. Demonstrates good verbal and written communication and organization skills.
5. Possesses and maintains current CPR certification/BLS Certification
6. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

JOB DESCRIPTION SUMMARY
The medical social worker is responsible for the implementation of standards of care for medical social work services.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

  • Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse.


  • Carries out social evaluations and plans intervention based on evaluation findings.


  • Maintains clinical records on all patients referred to social work.


  • Provides information and referral services for Organization patients and families/caregivers regarding practical and environmental needs.


  • Provides information to patients or families/caregivers and community agencies.


  • Serves as liaison between patients or families/caregivers and community agencies.


  • Maintains collaborative relationships with Organization personnel to support patient care.


  • Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.


  • Participates in in-service education programs.

  • Assists physician and other team members in understanding significant social and emotional factors related to health problems.

POSITION QUALIFICATIONS

JOB LIMITATIONS

The home health aide will not function in any manner viewed as the practice of nursing according to the State's Nurse Practice Act. Specifically, the home health aide will not administer medications, take physician's orders or perform procedures requiring the training, knowledge and skill of a nurse, such as sterile techniques.
