Upon receiving a Physician’s order, we will contact you to set an appointment for a member of our care team to visit your home.  

We may provide one or more of the following services:


SKILLED NURSING CARE 

Our Registered Nurses will manage your intermittent care while you recover from an illness or injury at home.  They will complete an initial assessment,  a reevaluation, and monitor and report your recovery, at home, to your physician.

  • Catheter Care
  • Treatment and Injections,

PHYSICAL THERAPY

​​Our Licensed Physical Therapists will assess your therapeutic and functional status. They will determine how safe your home or environment is to your condition and recommend changes to support your rehabilitation. They will instruct the patient family or caregivers in using rehabilitative and assistive devices.



Our Physical Therapy services include but are not limited:


  • Pain Management Techniques
  • Gait Training
  • Transfer Training
  • Fall Prevention Techniques
  • Mobility Device Assessment 
  • proper use of assistive devices such as crutches and wheelchairs 
  • exercises to improve posture motor functions and overall mobility
  • ​Home Safety evaluation


 

OCCUPATIONAL THERAPY

Our Occupational Therapists will evaluate your functional status such as muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, and work capacity. They will check your home for hazards and challenges to safe, independent living. They will develop treatment programs and establish goals for improved function and return to a former level of function.



SPEECH THERAPY


Our Speech Therapists will identify your  speech difficulties and establish a plan of care to improve your function.


Our Speech therapists may:


  • Administer speech therapy programs utilizing specialized therapeutic techniques and equipment.
  • Confer with referring physicians and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.





MEDICAL SOCIAL WORKER


​Our Medical Social Workers will assess your psychosocial status related to your illness and environment. They will help physicians and the care teams identify social and emotional factors related to your health problems.


Our Medical Social Worker services may:


  • connect patients to community resources
  •  provide home counseling and decision making
  • Social and Emotional Factors Assessment 
  • lead patient education on managing social, emotional, and financial challenges  





CERTIFIED HOME HEALTH AIDE  

Our Certified  Home Health Aides will advocate for the patient's personal care needs.  They can help you manage your everyday tasks and living needs. This service can include assistance in grooming, personal hygiene, and toileting.
