In some cases, patients can ask to be referred to our agency upon discharge.

While home health helps with recovery, it’s a patient’s right to refuse the service.
9. Does Eastbay have caregivers on staff?
We currently do not provide Caregiver services. But we are happy to help our patients find caregivers through other providers.
10. Does Eastbay provide transportation for its patients?
We do not have in-house transportation for our active patients at this time. But we are happy to help our patients find transportation through other providers.

Currently, we provide Home Health Services to patients in Solano County, Napa, Yolo and Contra Costa.

EastBay does not provide home health or home care to Pediatric patients. We only provide home health services to Adult and Elderly patients.
13. Does Home Health have a copay?
Traditional or Original Medicare patients do not have a copay.
